Class Participation

I understand that I must show up to all training classes without fail or I will be removed from the program without refund. This includes vacations.

All dogs are required to be up to date on the following vaccines. Please provide the expiration date for each vaccine’s

I confirm that my dog is up to date on Bordatella, DHPP, and Rabies vaccine.

Client Expectations: Check to acknowledge

Our training program involves the use of a Control Collar aka “choke collar”. This training tool is used in conjunction with proper hands on training that you are about to receive. Your trainer will go over proper use when returning for your first lesson to ensure proper use of control collar
Follow all instructions given and record yourself in daily accountability check-in videos every week and send directly to your trainer except on days with your trainer.
I as the client agree to meet with my trainer on a weekly basis and follow training program without modification and will provide weekly progress videos as part of my training plan homework.
As the client I am participating in this program with Strategic K9 Training freely and willingly and will not hold Strategic K9 Training responsible for any theft, damages and or injuries that may occur during my training lessons with Strategic K9 Training regardless of training location including but not limited to clients home, public locations and our training centers.

Acknowledgment and Release of Liability I, the undersigned ("Participant"), hereby acknowledge that I am voluntarily enrolling in dog training classes and related services, including but not limited to boarding, daycare, and any other activities offered by Strategic K9 Training ("Company"). I understand that while Strategic K9 Training will take all reasonable and appropriate measures to ensure the safety of all participants, dogs, and property during training sessions and other services, there are inherent risks associated with handling, training, and boarding dogs, including the risk of injury, illness, or death to myself, my dog, other dogs, and other persons. Assumption of Risk By signing this document, I voluntarily assume all risks associated with my participation and my dog's participation in any and all services provided by Strategic K9 Training, including but not limited to training, boarding, daycare, and any other activities. I acknowledge that dogs, by their nature, may be unpredictable, and I accept and assume full responsibility for any risks, injuries, or damages that may result from my dog's actions or the actions of other dogs or participants. Waiver of Claims In consideration of being permitted to participate in the activities and services provided by Strategic K9 Training, I, on behalf of myself, my heirs, next of kin, executors, administrators, legal representatives, successors, and assigns (collectively, the "Releasing Party"), hereby irrevocably waive, release, discharge, and covenant not to sue Strategic K9 Training, its owners, officers, directors, employees, agents, contractors, volunteers, successors, and assigns (collectively, the "Released Parties") from any and all claims, demands, causes of action, losses, damages, liabilities, costs, and expenses (including attorney's fees and court costs) of any nature, known or unknown, arising out of or related to my participation or my dog's participation in any activities or services provided by Strategic K9 Training. Indemnification I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, causes of action, losses, damages, liabilities, costs, and expenses (including attorney's fees and court costs) arising out of or related to my or my dog's participation in the activities and services provided by Strategic K9 Training, including but not limited to any claims or actions brought by third parties or other participants as a result of my dog's behavior or actions. Acknowledgment of Risks and Consultation I acknowledge that I have been fully informed of the risks associated with participating in the activities and services provided by Strategic K9 Training, including the risks of injury, illness, or death to myself, my dog, or others. I have had the opportunity to discuss any concerns I may have regarding these risks with representatives of Strategic K9 Training and have made an informed decision to proceed with participation. Medical Treatment and Emergency Care In the event that my dog requires medical treatment or emergency care during their participation in any activities or services provided by Strategic K9 Training, I authorize the Company and its representatives to seek veterinary care on behalf of my dog. I agree to be solely responsible for all costs and expenses associated with such care. Consent for Media Use I hereby consent to the use of photographs, video, or other audiovisual recordings of my dog taken during their participation in any activities or services provided by Strategic K9 Training for educational, promotional, or marketing purposes. I understand that such media may be used in various forms, including but not limited to online and print publications, social media, and advertising materials. Program Fees and Additional Costs I acknowledge that certain fees, including but not limited to promotional examination fees, equipment costs (e.g., training gear, additional uniforms), and tournament fees, are not included in the standard program pricing offered by Strategic K9 Training. I agree to pay any such additional fees as applicable. Legal Binding and Enforceability This Acknowledgment and Release of Liability shall be legally binding upon the Releasing Party and shall be construed in accordance with the laws of the state in which Strategic K9 Training operates. In the event that any provision of this document is found to be unenforceable or invalid by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect. Modification and Waiver No modification or waiver of any provision of this Acknowledgment and Release of Liability shall be valid unless expressly agreed to in writing by both the Released Parties and the Releasing Party. Any failure to enforce any provision of this document shall not constitute a waiver of such provision or any other provision. Entire Agreement This Acknowledgment and Release of Liability constitutes the entire agreement between the Releasing Party and the Released Parties concerning the subject matter herein and supersedes any prior agreements, understandings, or representations, whether oral or written, regarding the same.

By checking this box, you acknowledge that you have read, understood, and agree to the terms and conditions outlined above. Your participation in the activities and services provided by Strategic K9 Training is contingent upon your acceptance of these terms.

Refund Policy: Check to acknowledge

Once training has started Strategic K9 Training cannot provide any refund of any kind. If program cancelled prior to first lesson we will refund your entire tuition cost however evaluation fee cannot be refunded. Our trainers reserve the right to refuse services or dismiss client from program if client is not following training plan including but not limited to repeat cancellation of appointments, not completing homework assignments, unsafe behavior and any behavior that is inappropriate.
I understand fully what is expected of me and will follow all instructions given to me by my trainer. I agree to the above and will communicate with my trainer when needed with questions or concerns and hold myself accountable for the outcome of my training I am enrolling my dog in.

I hereby understand that there are four classes that cannot be skipped unless an emergency or accepted by head trainer. If I skip a class I acknowledge and accept that I may be removed from class without refund.
